Medical Order/ Prescription Requirement
The CES Ultra may be purchased anywhere in the world without a prescription. If you are ordering from outside the U.S., no prescription is required.
In the U.S., however, the CES Ultra may be purchased only on the order of a licensed healthcare provider. The term “licensed healthcare provider” (LHP) includes, but is not limited to: Medical Doctors, Osteopaths, Chiropractors, Naturopaths, Acupuncturists, Psychologists, and Psychiatrists. The actual scope of professional licensing varies from state to state.
If You Have a Licensed Healthcare Provider
Simply download and print out the prescription form, and present it to your licensed healthcare provider to sign. Then simply provide the signed form to the CES Ultra seller/distributor at the time of purchase.

Your healthcare practitioner may not be familiar with CES. If they should require more information, we recommend that you refer them to this website. You may also go to our Frequently Asked Questions page and print it out for your practitioner. We always welcome their phone calls and are happy to have a conversation with them on the subject.
If You Do Not Have a Licensed Healthcare Provider
If you do not have a licensed healthcare provider (LHP), or if your LHP is unfamiliar with CES, you can consult with a recommended LHP ($20 charge paid directly to the LHP). Simply check the box at the bottom of the checkout page requesting a consultation with an LHP. If the LHP determines that the CES Ultra is appropriate for you, a written prescription will be provided to us, after which we will process your order. Call us now at 1-425-222-0830 for details.
